It must have been understood from the above discussion why the risk of severe consequences increases when dengue is infected for the second time compared to the first time. Almost everyone (especially in Dhaka) has contracted dengue at least once in the last few years, many have had no problems. So this infection is at least number two infection for most people. This may also be a reason for the extreme severity of the dengue outbreak.

However, it also happens that someone starts bleeding at the first infection or goes into shock due to loss of water from the blood vessels. Some are still walking around after being infected a second time with a different serotype. The reasons for such differences are not yet fully understood. However, research suggests that genetic differences may be responsible for this. Apart from this, the severity of disease symptoms varies depending on the serotype.

Serotype testing requires RNA-based PCR, which is relatively expensive and only available in two or four specialized institutions in Bangladesh. Apart from that, there is a need for detailed research on the reasons behind the strange things like myocarditis in dengue patients without any symptoms.

While there are many antibiotics for bacteria, there are very few specific drugs for viral diseases. In most cases vaccination is the only hope against the virus. Vaccines usually work by stimulating the immune system to produce antibodies that specifically bind to that particular pathogen.

Or the antibody molecules are often injected directly into the body as a vaccine. Developing a dengue vaccine poses a particular challenge. If antibodies to all serotypes are not activated simultaneously, the patient may become more ill rather than immunosuppressed due to ADE.

For this reason, there is only one World Health Organization and FDA approved vaccine called Dengavexia, which can protect against all four serotypes simultaneously. It applies to 9 to 16-year-olds who have had dengue once before. Unfortunately, this vaccine is available in only 20 countries, which does not include Bangladesh.
